The **Ontario Ministry of Health and Long-Term Care** is a Canadian provincial government department of the [Government of Ontario](https://www.whiteroseintelligence.com/Government-of-Ontario). It is a joint agency made up of the Ministry of Health and the Ministry of Long-Term Care.

## History

On April 27, 2021, Dr. Patrick Phillips received a letter from Glenn Corneil, acting Medical Officer of Health and CEO of Temiskaming Health Unit, rejecting five [COVID-19 vaccine adverse event](https://www.whiteroseintelligence.com/Adverse-events-associated-with-COVID-19-vaccines) reports. Corneil directed Phillips to refer to the [December 2020 version](https://web.archive.org/web/20210416015711/http://www.health.gov.on.ca/en/pro/programs/publichealth/oph%5Fstandards/docs/aefi%5Fcd.pdf) of the Ministry of Health's document titled "Infectious Diseases Protocol Appendix B: Provincial Case Definitions for Diseases of Public Health Significance - Disease: Adverse Events Following Immunization (AEFIs)."[\[1\]](#fn1)
